About The Position

This is a 13-week travel assignment for a Registered Nurse specializing in Geriatric care at an acute care hospital in Glendale, CA. The role involves providing skilled nursing care to geriatric patients, collaborating with a multidisciplinary team, and managing care plans. The nurse will act as a liaison between patients, families, and healthcare professionals, assess complex patients, facilitate transfers, and support documentation for regulatory activities. The position offers 36 guaranteed hours per week, with a day shift schedule (5x8).

Requirements

  • Valid Registered Nurse license in the state of practice (California)
  • Basic Life Support (BLS) certification required
  • RN Case Management (CM) certification required
  • Minimum of two years of recent experience in the field; care management or IP CM experience preferred
  • Ability to work in a fast-paced hospital environment and to manage a heavy workload
  • Eligible to work in the United States without sponsorship

Responsibilities

  • Provide skilled nursing care to geriatric patients in an acute care setting, collaborating with physicians and the multidisciplinary team
  • Lead and coordinate the care plan, monitor patient progress, and adjust interventions as the clinical picture evolves
  • Serve as liaison among the patient, family, nursing staff, physicians, and other care team members to optimize outcomes and discharge planning
  • Assess complex patients, including high ED utilizers, chronic pain patients, and those with substance use or housing challenges, ensuring appropriate resource use
  • Facilitate transfers to appropriate levels of care and ensure continuity of information for post-discharge planning
  • Support regulatory and accreditation activities by documenting care, outcomes, and quality indicators
