About The Position

This role focuses on coordinating patient care across the continuum to ensure quality outcomes and efficient resource utilization. The Registered Nurse – Case Manager will facilitate discharge planning and transitions of care for patients and families. Responsibilities include conducting utilization reviews, case management assessments, and care coordination activities. The position requires collaboration with physicians, social workers, nursing staff, and interdisciplinary healthcare teams to monitor patient progress, identify barriers to discharge or treatment plans, and ensure compliance with regulatory requirements, payer guidelines, and organizational policies. The role involves advocating for patient needs while promoting safe and effective care delivery, maintaining accurate and timely documentation, and supporting patients and families through education and resource coordination.

Requirements

  • Active Registered Nurse (RN) License.
  • Current BLS Certification.
  • Recent Case Management experience required.
  • Strong knowledge of discharge planning, utilization review, and care coordination.
  • Excellent communication, organizational, and problem-solving skills.
  • Ability to work collaboratively with multidisciplinary healthcare teams.

Nice To Haves

  • Acute care hospital case management experience.
  • Experience with utilization management and payer regulations.
  • Familiarity with electronic medical record (EMR) systems.
  • Certification in Case Management (CCM or ACM) preferred.
