About The Position

This role involves architecting comprehensive care plans to bridge the gap between hospital and home, ensuring alignment with patient values and recovery goals. The Registered Nurse Case Manager will advocate for patients by identifying and removing systemic barriers to care, ensuring they have the essential resources for a safe transition. This position will also orchestrate clinical communication across the multidisciplinary team for seamless execution of health plans and champion documentation integrity by serving as a clinical resource to ensure medical records accurately reflect the necessity of services and the complexity of the patient's journey.

Requirements

Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required.
Licensure from the Wisconsin Board of Nursing OR current home state license if considered multi-state/Compact State.
Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.

Nice To Haves

BLS Provider preferred. American Heart Association or American Red Cross accepted.
Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.
