About The Position

Dove Healthcare is hiring a Registered Nurse Case Manager for its St. Croix Falls location. The role is suited to an RN who wants to mentor, train, and educate caregivers while coordinating and overseeing patient care. The case manager works with employees, patients, family members, and other professionals to coordinate person-centered care. This is a limited-term position with the potential to become permanent.

Requirements

• Current licensure in the State of Wisconsin as a registered nurse (RN).
• Degree from an accredited school of nursing.
• Previous nursing experience.

Responsibilities

• Supervise the day-to-day activities of assigned residents and employees.
• Coordinate, plan, implement, and evaluate the nursing services provided to each resident using clinical competence.
• Review and audit compliance with each patient's plan of care and changing needs.
• Provide a safe, comfortable, and home-like environment.
• Participate in an on-call rotation.
