About The Position

Hospice of the West is seeking a per diem Hospice Registered Nurse Case Manager to provide end-of-life care throughout the Casa Grande territory. The RN Case Manager delivers hands-on hospice nursing care, coordinates services, and supports patients and families throughout their hospice journey as part of an interdisciplinary care team. The role focuses on compassion, clinical excellence, comfort, dignity, and quality of life.

Requirements

• Hold a current, unencumbered Registered Nurse license.
• Have at least one year of RN experience in a medical-surgical or acute-care setting.
• Be proficient in typing and various computer programs, including electronic medical records.
• Maintain current CPR certification if required by the state.
• Have a current driver's license, vehicle insurance, and access to dependable transportation.

Responsibilities

• Respond to clinical referral information and address inquiries and requests.
• Partner with the Clinical Director to assess client eligibility and suitability for hospice services.
• Provide direct nursing care and support patient comfort and quality of life.
• Deliver nursing care using established principles and techniques, including care that eases pain and provides comfort.
• Educate and advocate for patients and families, encouraging their participation in personalized care plans aligned with their goals.
• Share responsibility for call duties.
