About The Position

Continuum Hospice is seeking a Registered Nurse Case Manager to provide compassionate hospice care to patients and families in the community, with a territory covering Santa Cruz and San Jose. The RN Case Manager delivers hands-on nursing care, coordinates services, and supports patients and families throughout the hospice journey as part of an interdisciplinary care team. Requirements

Hold a current, unencumbered Registered Nurse license.
Have a bachelor's degree in nursing from a program accredited by the National League for Nursing, or one year of professional nursing experience within the last three years.
Be proficient with computers, typing, and various programs, including electronic medical records (EMR).
Maintain current CPR certification.
Have a current driver's license, vehicle insurance, and access to dependable transportation.

Hospice or home health experience is preferred but not required.

Responsibilities

Respond to clinical referral information and address inquiries and requests with care and expertise.
Partner with the Clinical Director to assess client eligibility and suitability for hospice services.
Coordinate care and provide direct nursing support to promote patient comfort and quality of life.
Deliver nursing care using established principles and techniques to help ease pain and provide comfort.
Educate and advocate for patients and families, encouraging their participation in personalized care planning.
Share responsibility for on-call duties.
