About The Position
St. Croix Hospice is seeking a Registered Nurse Case Manager to provide compassionate, individualized, holistic care to patients in their homes. The role is central to patient care and involves collaborating with the care team to create personalized plans of care, leading end-of-life conversations, and supporting effective symptom management with empathy and clinical excellence. The position offers meaningful patient connections and autonomy in nursing practice.
Requirements
- Associate, bachelor's, or master's degree from an accredited nursing program and Registered Nurse (RN) qualification.
- At least 1 year of recent acute care experience.
- Reliable transportation, a valid driver's license, and proof of auto insurance.
Nice To Haves
- Hospice or home health experience.
Responsibilities
- Complete initial and ongoing comprehensive assessments to determine hospice needs.
- Develop individualized care plans with patients and families, updating them as needs evolve.
- Administer medications and treatments as prescribed.
- Educate patients and caregivers on care techniques, medications, diet, and symptom management.
- Rotate on-call duties and ensure seamless care transitions.
