About The Position

St. Croix Hospice is seeking a Registered Nurse Case Manager to provide individualized, holistic care to patients in their homes. The Case Manager collaborates with the care team to develop personalized plans of care, leads end-of-life conversations, and supports effective symptom management with empathy and clinical excellence. The role calls for compassionate patient connections and autonomy in nursing practice.

This is a full-time position based in Oakdale, Minnesota, with a stated schedule of 8:00 a.m. to 5:00 p.m. and on-call expectations. The role includes 100% travel and requires reliable transportation, a valid driver’s license, and proof of auto insurance. The annual base salary range is $80,000–$96,000. A $5,000 sign-on bonus is advertised.

Requirements

• Associate, bachelor’s, or master’s degree from an accredited nursing program and registered nurse qualification.
• At least 1 year of recent acute care experience.
• Reliable transportation, a valid driver’s license, and proof of auto insurance.

Responsibilities

• Complete initial and ongoing comprehensive assessments to determine hospice needs.
• Develop individualized care plans with patients and families, updating them as needs evolve.

• Educate patients and caregivers on care techniques, medications, diet, and symptom management.
• Rotate on-call duties and ensure seamless care transitions.
• Collaborate with the care team to provide individualized, holistic support to patients in their homes.
• Lead meaningful end-of-life conversations and support exceptional symptom management.
