About The Position
St. Croix Hospice is seeking a Registered Nurse Case Manager to provide one-to-one, whole-person care to patients and families in their homes. Working with an integrated care team, the nurse coordinates individualized care plans, supports families through complex decisions, and provides comfort with professionalism, empathy, and respect. The role offers meaningful patient connections and autonomy in nursing practice. The position is based in Newburgh, Indiana, is full time, and includes on-call expectations. Requirements
- Associate, bachelor's, or master's degree from an accredited nursing program (RN).
- At least 1 year of recent acute care experience.
- Reliable transportation, a valid driver's license, and proof of auto insurance.

Responsibilities
- Complete initial and ongoing comprehensive assessments to determine hospice needs.
- Develop individualized care plans with patients and families, updating them as needs evolve.
- Administer medications and treatments as prescribed.
- Educate patients and caregivers on care techniques, medications, diet, and symptom management.
- Rotate on-call duties and ensure seamless care transitions.
- Coordinate individualized care with an integrated care team.
- Support families through complex decisions and provide comfort with professionalism, empathy, and respect.
