About The Position
St. Croix Hospice provides compassionate end-of-life care, guiding patients and families with dignity, comfort, and respect. The Registered Nurse Case Manager provides one-to-one, whole-person care to patients and families in their homes. Working with an integrated care team, the nurse coordinates individualized care plans, supports families through complex decisions, and provides professional, empathetic care. The position is based in Frederic, Wisconsin, and involves 100% travel. The schedule is 8:00 a.m. to 5:00 p.m., with on-call expectations.
Requirements
- Associate, bachelor's, or master's degree from an accredited nursing program (RN).
- At least one year of recent acute care experience.
- Reliable transportation, a valid driver's license, and proof of auto insurance.
Nice To Haves
- Hospice or home health experience.
Responsibilities
- Complete initial and ongoing comprehensive assessments to determine hospice needs.
- Develop individualized care plans with patients and families and update them as needs evolve.
- Administer medications and treatments as prescribed.
- Educate patients and caregivers on care techniques, medications, diet, and symptom management.
- Rotate on-call duties and ensure seamless care transitions.
- Coordinate care with an integrated care team and support families through complex decisions.
