About The Position

St. Croix Hospice is seeking a Registered Nurse Case Manager to provide one-to-one holistic support to patients and families in their homes. The role partners with an interdisciplinary team to develop individualized care plans, guide end-of-life conversations, and promote patient comfort with professionalism and empathy. The position offers meaningful patient connections and autonomy in practice. It is full-time, based in Storm Lake, Iowa, with a stated schedule of 8:00 a.m. to 5:00 p.m. and on-call expectations. Travel is listed as 100%.

Requirements

• Associate, bachelor's, or master's degree from an accredited nursing program (RN).
• At least 1 year of recent acute care experience.
• Reliable transportation, a valid driver's license, and proof of auto insurance.
• Ability to pass a DHS background study.

Nice To Haves

• Hospice or home health experience preferred.

Responsibilities

• Complete initial and ongoing comprehensive assessments to determine hospice needs.
• Develop individualized care plans with patients and families, updating them as needs evolve.
• Administer medications and treatments as prescribed.
• Educate patients and caregivers on care techniques, medications, diet, and symptom management.
• Rotate on-call duties and ensure seamless care transitions.
• Partner with an interdisciplinary team to support patients and families.
• Guide end-of-life conversations and help ensure patient comfort.
