About The Position

St. Croix Hospice is seeking a Registered Nurse Case Manager to provide compassionate, individualized, holistic care to patients in their homes. The role serves as a cornerstone of patient care, working with the care team to develop personalized care plans, guide meaningful end-of-life conversations, and provide symptom management with empathy and clinical excellence. The position is based in Sedalia, Missouri, is full-time, and includes on-call expectations. The stated schedule is 8:00 a.m. to 5:00 p.m. A $5,000 sign-on bonus is advertised.

Requirements

• Associate, bachelor's, or master's degree from an accredited nursing program and Registered Nurse (RN) qualification.
• At least 1 year of recent acute care experience.
• Reliable transportation, a valid driver's license, and proof of auto insurance.

Nice To Haves

• Hospice or home health experience is preferred.

Responsibilities

• Complete initial and ongoing comprehensive assessments to determine hospice needs.
• Develop individualized care plans with patients and families, updating them as needs evolve.
• Administer medications and treatments as prescribed.
• Educate patients and caregivers on care techniques, medications, diet, and symptom management.
• Rotate on-call duties and ensure seamless care transitions.
• Collaborate with the care team to support personalized patient care.
• Lead meaningful end-of-life conversations and provide symptom management.
