Requirements

Current, unrestricted state RN licensee
1+ year experience in a clinical care setting as a nurse
Able and willing to travel within branch/office coverage area.
Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
Must be able and willing to travel 50%

Nice To Haves

Home health experience is helpful, but not required.
Associates Degree or Bachelor’s Degree in Nursing is preferred
Previous Home Health or Hospice Experience is preferred
Previous experience with HomeCare HomeBase is preferred

Responsibilities

Assess patients and develop individualized care plans.
Provide skilled nursing care in patients’ homes.
Teach patients and caregivers about medications, treatments, and managing their health.
Coordinate care with physicians and the interdisciplinary team.
Complete timely, accurate clinical documentation.
Care for patients in Youngstown, Warren, Cortland, Poland, Boardman, and surrounding communities in Trumbull and Mahoning counties.
