About The Position

Registered Nurse Case Managers provide medical assistance while listening attentively to patients’ needs and honoring their wishes. They build trusting relationships with patients and families and serve as a central point of communication within an interdisciplinary team of holistic care providers.

In hospice care, nurses help patients spend their final days in ways that reflect who they are and what matters to them. The role involves providing compassionate care, honoring patients’ wishes, and supporting dignity and comfort for patients and their families during a difficult journey.

Requirements

• At least 6 months of prior skilled nursing experience.
• An active, unencumbered state Registered Nurse license.
• A current driver’s license and auto liability coverage.
• Interest and ability in providing patient care in homes and facilities.

Responsibilities

• Provide patient care in homes and facilities.
