About The Position

Coordinate the overall interdisciplinary plan of care across the continuum from admission to discharge, integrating patient and family preferences and values. Monitor patient care through assessments, evaluations, and record reviews, serving as a resource for proper medical documentation, service appropriateness, and post-discharge treatment options. Advocate for necessary resources, eliminate care barriers, and maintain ongoing dialogue with supervisors and healthcare providers for effective plan implementation. Oversee patient progress and care quality while facilitating clear communication among all members of the healthcare team.

Requirements

Registered Nurse credentialed from the Illinois Board of Nursing obtained prior to hire date or job transfer date.
Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.

Nice To Haves

BLS Provider. American Heart Association or American Red Cross accepted.
Previous in patient discharge experience highly desired
