RN - Referral Intake Nurse
About The Position
The Referral Intake Nurse initiates, screens, and triages referrals for Home Health, Hospice, and/or Palliative Care. The nurse maintains relationships with referral sources and insurance companies, educates referral sources about the types of patients served and service eligibility requirements, and oversees and reviews referrals taken by Intake Support Specialists. The role receives and documents referrals made by phone or fax from hospitals, clinics, other facilities, and families. It assesses patients’ clinical needs using information from referral sources and available medical records, determines eligibility against program criteria, verifies skilled-care orders with physicians and obtains histories and physicals as needed, and gathers insurance information for verification. The nurse also obtains authorization for initial visits when appropriate and required by the insurer. Candidates must live in Washington state.
Requirements
- Current State of Washington professional nursing license.
- Current BLS Healthcare Provider certification by date of hire.
- Two years of Home Health, Hospice, or other Home Care experience.
- Prior marketing, sales, and/or community outreach experience.
- Current Washington State driver's license and proof of insurability if the position requires regular driving.
- Working knowledge of Medicare COP and JC Standard.
- Familiarity with third-party insurance and Medicare HMOs.
- Must live in Washington state.

- Master's degree in nursing.
Responsibilities
- Receive and document Home Health, Hospice, and/or Palliative Care referrals made by phone or fax from outside sources, including hospitals, clinics, other facilities, and families.
- Assess patients’ clinical needs using information from referral sources and available medical records.
- Determine patient eligibility for Home Health, Hospice, and/or Palliative Care services against program criteria.
- Verify skilled-care orders with physicians and obtain histories and physicals from physicians as needed.
- Obtain insurance information from referral sources and forward it to the Business Office insurance verifier.
- Obtain insurance authorization for initial visits when appropriate and required by the insurer.
- Maintain relationships with referral sources and insurance companies.
- Educate referral sources about the types of patients served and eligibility requirements.
- Oversee and review referrals taken by Intake Support Specialists.
