About The Position
In this position, you will be a part of a team that goes out into the community, providing mental health support services to adults who live with serious mental illnesses.
Requirements
- Bachelor’s degree in Social Work, Human Services, Psychology, or a related field
- A valid driver’s license and reliable transportation.
- A valid QMHP certification or Higher.
Responsibilities
- Completing treatment plans and daily progress notes.
- Carrying out rehabilitation and support functions.
- Skills restoration and care coordination.
- Supporting the individual with required services within the scope of their credentialing and expertise.
- Attending treatment team meetings, as available.
- Documenting all interactions with client.
- Having a working knowledge of DMAS, DBHDS and MCO rules and regulations.
- Ensuring all of their personnel documents are accurate and up to date and immediately notifying Human Resources of any changes that could result in their inability to perform their job functions.
