About The Position

PATH is seeking candidates passionate about helping others make a positive change in their lives to join our Interim Housing Team as the Program Manager at the RHC location. As part of the Interim Housing team, the Program Manager is responsible for the program's day-to-day operations and supervision of service delivery staff. The incumbent will collaborate with both internal and external departments to enhance accessibility and service delivery for program participants. The incumbent is responsible for developing and ensuring adherence to programmatic policies and procedures. The incumbent will also be responsible for identifying and providing trainings.

Requirements

  • Current and active licensure with the California Board of Behavioral Sciences (e.g., LCSW, LMFT, LPCC).
  • Master’s degree in social work or equivalent behavioral health degree eligible for licensure through the CA Board of Behavioral Sciences (e.g., MSW, MFT)
  • Two (2) years of experience in healthcare, case management, homeless services or working with vulnerable populations.

Nice To Haves

  • Three (3) years of experience in healthcare, case management, homeless services or working with vulnerable populations.
  • One (1) year of supervisory experience in homeless services or related social services field strongly preferred.

Responsibilities

  • Provide leadership to the multidisciplinary teams, ensuring continuity of care and services between PATH agency resources and partnering agencies
  • Provide clinical oversight of staff and ensure that all program participants receive the services as identified in each participant service plan.
  • Create and work within a team environment to ensure staff is knowledgeable and familiar with contract expectations, case management and supervisorial expectations and ensure staff is delivering high quality care.
  • Provide individual and team supervision, support, and performance evaluations for assigned staff
  • Ensure that services emphasize trauma informed care, client safety, and harm reduction interventions.
  • Be available to respond to emergency calls and situations as required by the contract and organization
  • Assist in the screening, assessment, and enrollment of clients in a range of services
  • Ensure all paperwork and data collection is complete, timely, accurate, and current in agency records and electronic databases through ongoing audits of electronic databases and chart reviews
  • Ensure adherence to funding source contracts and performance standards; prepare written reports on program activities as assigned and as required by funding sources
  • Provide information to department supervisors and leaders
  • Effectively communicates project expectations to team members
  • Coordinate and collaborate with community partners.
  • Oversee employee performance evaluations, conduct regular and routine supervision meetings, ensure timely review and management of timesheets and attendance issues in a consistent manner, provide recognition and corrective action as needed, address concerns, and facilitate problem resolutions.
  • Represent the company in employment-related matters as part of supervisory responsibilities, including participation in court proceedings, unemployment hearings, and other relevant legal or regulatory processes.
  • Attend and actively engage in all required training sessions as assigned by the company and apply learned skills and knowledge to daily tasks and responsibilities.
  • Attend and actively engage in all required staff meetings and events on and off-site (when applicable).
