About The Position

Key Rehab is seeking a full-time Program Coordinator, specifically an Occupational Therapist (OT) or Physical Therapist (PT), to join their team in Conrad, IA. This role is based in a Skilled Nursing Facility (SNF) setting and offers competitive rates of up to $46+ per hour. A sign-on bonus of $3,000 is available for qualified applicants. The Program Coordinator acts as a liaison between Key Rehab and community staff, aiming to reduce administrative burdens on clinicians and support their ability to deliver therapy services. This position also involves coordinating staffing and daily scheduling to meet performance expectations under the guidance of a Regional Supervisor.

Requirements

Completion of an accredited PT, OT, or SLP program.
Current PT, OT, or SLP license to practice in the state(s) assigned.
Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP).
CPR, as required by community.
Valid driver license and car insurance as required by state law.
Other items as required by federal, state and local standards.
