About The Position

Key Rehab is seeking a full-time Program Coordinator-OT/PT/SLP to join their team in the Mt. Pleasant, IA area, specifically within a Skilled Nursing Facility (SNF) setting. This role involves providing excellent care and serving as a liaison between Key Rehab and community staff. The Program Coordinator is crucial for minimizing administrative burdens on clinicians, allowing them to focus on delivering therapy services. Additionally, this position is responsible for coordinating staffing and daily scheduling to meet key performance indicators and expectations, operating under the direction of a Regional Supervisor.

Requirements

  • Completion of an accredited PT, OT, or SLP program
  • Current PT, OT, or SLP license to practice in the state(s) assigned
  • Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP)
  • CPR, as required by community
  • Valid driver license and car insurance as required by state law
  • Other items as required by federal, state and local standards

  • Prior experience in long term care setting
  • Prior experience in service coordination
  • Strong Organizational and Problem-Solving Skills: Ability to efficiently manage tasks and address challenges with effective solutions.
  • Flexibility and Team Collaboration: Adaptable to changing situations and committed to working seamlessly within a team environment.
  • Exceptional Communication: Outstanding oral and written communication skills for clear and effective interaction with patients and colleagues.
  • Experience with Geriatric Population: Prior experience working with elderly patients, demonstrating an understanding of their unique needs.
  • Knowledge of Medicare Billing Regulations: Familiarity with current Medicare billing practices and guidelines.

  • Providing therapy under their disciplinary scope of practice (OT, PT, or SLP)
  • Serving as the liaison between Key Rehab and community staff
  • Minimizing administrative burden and providing support to allow clinicians to deliver therapy services
  • Coordinating staffing and daily scheduling to meet key performance indicators and expectations under the direction of a Regional Supervisor
