About The Position

Empath Home Health, a member of Empath Health, is hiring a Professional Relations Liaison to support Hillsborough County. This relationship-focused and educational role serves as a consultant to referral sources, educating them about Empath Health’s home health services and helping improve access to home health and related programs. The Liaison develops expertise in home health services and explains their benefits and scope to the professional community. The role also identifies and facilitates opportunities for marketing, referrals, community outreach, education, and collaboration. Empath Home Health provides skilled nursing, therapy, and personal care support to help patients recover, maintain independence at home, and receive care as part of a connected continuum of services.

Requirements

• At least 2 years of outreach in home health, professional relations, and/or sales experience in home health.
• Ability to work unsupervised with a high degree of discretion and independent judgment.
• Effective communication and relationship-building skills.

Responsibilities

• Educate referral sources about Empath Health home health services.
• Educate the professional community about the benefits and scope of home health services.
• Identify and facilitate opportunities for marketing, referrals, community outreach, and home health program education.
• Identify and facilitate collaborations that can improve access to home health and related programs.
