About The Position

Heartline Staffing is hiring a Primary Care Physician for a physician-led, mission-driven organization providing senior-focused, full-risk Medicare Advantage care. The organization uses an outcomes-focused, value-based model in which physicians build trusting relationships with older adults and help them change behaviors and work toward short- and long-term health goals. Physicians act as health coaches, lead a dedicated care team, and manage a panel of up to 450 patients. The practice model includes 18–21 appointment slots per day; new patients count as two slots and follow-up patients as one. The care team includes a medical assistant, care facilitator, and care coordinator. Physicians may supervise or collaborate with Advanced Practice Practitioners and are accountable for the care provided. Performance measures include patient admissions per thousand, patient experience (CGCAHPS), clinical gap closures, and medical cost measures. The position requires availability for walk-in visits and timely responses to patient calls and messages. The physician independently provides care for older adults with acute and chronic illnesses and remains accountable for patient care and outcomes, consulting specialists as needed. Responsibilities include assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation for acute and complex chronic needs. The physician leads care for patients able to come to the office and coordinates with transitional care teams, case managers, and acute and transitional-care physicians for patients in hospital, skilled nursing, long-term care, or homebound settings. The role also includes participation in risk and quality management programs and clinical and other required meetings; contributing to center management and covering for other providers when needed; supporting patient and provider recruitment; and helping foster a positive, accountable center culture alongside center leadership. Travel includes national travel for initial two-week training, local travel within the market as needed, and occasional regional and national travel for recruiting or training. The base salary is $221,141–$315,915 annually. Final compensation depends on experience, education, geographic location, and other relevant factors. The position may also be eligible for bonuses or commissions. The employer offers a comprehensive benefits package and career development and advancement opportunities.
