ChenMed•Arlington, TX

About The Position

ChenMed is a physician-led, mission-driven primary care organization focused on providing affordable, high-quality care to seniors. The Primary Care Physician (PCP) independently manages acute and chronic illnesses in older adults and is accountable for the care and outcomes of a panel of up to 450 patients. The PCP builds trusting relationships, coaches patients toward health goals, and simplifies care plans to support actionable behavior change. This is an outcomes-focused, value-based role. PCPs are measured against patient admissions, appointment-slot utilization, patient experience, clinical gap closures, and medical-cost measures. They work with center and market leadership and care teams toward established goals, champion a positive culture, and participate in center management. The PCP works independently and collaboratively with other healthcare professionals, follows organizational and regulatory standards, and participates in quality and risk management programs and required meetings. The PCP leads a care team and coordinates care with specialists, hospitalists, transitional-care teams, case managers, and other resources as appropriate. The role also includes supervising or collaborating with an Advanced Practice Practitioner and remaining accountable for that practitioner’s actions. The position requires availability to patients, local travel as needed, and occasional regional or national travel for recruiting or training. Evening, weekend, or holiday work may be needed.
