About The Position

Heartline Staffing is hiring a Primary Care Physician for a physician-led organization focused on improving the health and well-being of seniors. The physician will provide direct patient care and assessments primarily in an ambulatory health care center, with occasional care in acute care, nursing home, skilled nursing facility, and home settings depending on the assignment. As a member of the clinical operations team, the physician will provide geriatric assessment, diagnosis, treatment, care planning, health education, and referrals. The role includes participation in clinical rounds, conferences, and quality meetings. The schedule details will be shared during a recruiter conversation. The position offers a base salary of $221,141–$315,915 annually, with possible eligibility for bonuses or commissions. Requirements

MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty.
Current, active Florida medical license or ability to obtain one.
Board eligible in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty.
Current DEA number for Schedule II–V controlled substances.
BLS certification from the American Heart Association or American Red Cross within the first 90 days of employment.
Ability to function independently and in collaboration with other health care professionals.
Commitment to evidence-based practice, quality improvement, and practice innovation.
Leadership skills to guide change and foster collaboration with patients, care teams, and stakeholders.
Strong oral and written communication skills.
Ability to coach patients and caregivers toward positive behavioral change.
Once board certified, maintain certification through MOC, CME, and/or board exams as required.

Nice To Haves

Board certification in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty.

Responsibilities

Function independently as a primary care practitioner within a patient care team.
Independently assess acute and non-acute clinical problems.
Perform and document physical assessments and patient histories, analyze trends, and develop and implement patient management plans.
Plan care based on knowledge of the patient population, considering cultural background, level of understanding, personality, and support systems; serve as a patient advocate.
Write admission, transfer, and discharge orders.
Order and interpret laboratory and diagnostic studies, and order appropriate medications and treatments.
Refer patients for specialty consultation and case management as indicated.
Document care through in-depth progress notes and summaries.
Participate in patient care rounds and conferences and communicate management strategies to the care team.
Recognize situations requiring immediate attention and initiate life-saving procedures when necessary.
Work with physicians, nurses, therapists, social workers, and caregivers to transition patients to a lower level of care when medically appropriate.
Facilitate patient and family conferences about treatment goals, education, and needs.
Support informed decisions about goals of care, palliative care, and hospice.
Manage resource utilization and reimbursement for services.
Participate in risk and quality management programs and collaborate to improve care delivery.
