About The Position

Aledade is hiring a Practice Transformation Specialist (PTS) to support its Florida practices. The role is the primary interface between headquarters and partner providers, helping identify practice needs and improve patient care while reducing costs. The PTS implements products and ACO initiatives with direction from Market Leadership and supports practices in meeting goals established in mutually developed Practice Transformation Plans. This is primarily a remote role, but regular in-person check-ins with practices are important. Travel is typically approximately once per month per practice, with schedules varying according to practice needs. Candidates must be based in Florida. The PTS trains practice staff and physicians and providers on processes and technologies, in coordination with Aledade’s Value Based Care Curriculum, and continually assesses training needs. The role also builds lasting relationships with providers and healthcare system executives to support financial and quality performance, and listens to practice concerns while sharing relevant information with managers and leads to identify opportunities for improvement.
