About The Position

Join Fortis Care in partnership with Sadie G. Mays Health & Rehabilitation Center, a historic skilled nursing institution with over 70 years of service. We are seeking a mission-driven Nurse Practitioner (NP) or Physician Assistant (PA) passionate about geriatric and post-acute care to improve the quality of life for vulnerable patients. In this role, you will be the primary clinical provider dedicated to a single facility, allowing for stability and the development of meaningful relationships with patients, families, and staff. This position is designed for work-life balance and clinical focus, featuring a Monday-Friday, day shift schedule with no night or weekend call, and a single-site dedicated focus. You will manage acute and chronic conditions, guide post-hospital transitions, and collaborate with facility leadership to deliver compassionate, high-touch clinical care.

Requirements

  • Current, unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license in the state of Georgia.
  • National certification (AGNP, FNP, Adult, or PA-C) with an active, unrestricted DEA license.
  • Minimum 1–2 years of experience as an APP.
  • Strong collaborative communication skills to engage effectively with nursing staff, attending physicians, and resident families.

  • Strong preference for post-acute, skilled nursing (SNF), or geriatric care experience.

Responsibilities

  • Provide compassionate, evidence-based medical care to patients at Sadie G. Mays, including assessment, diagnosis, treatment, and ongoing management.
  • Direct complex chronic disease management and sub-acute rehab care, reducing unnecessary hospital readmissions.
  • Lead interdisciplinary care planning alongside facility nurses, therapists, social workers, and families to support patient-centered goals.
  • Support smooth transitions of care through post-admission evaluations, comprehensive medication reconciliations, and family conferences.
  • Complete timely, accurate clinical documentation in the facility EHR.
  • Serve as a clinical champion for quality improvement initiatives that directly enhance resident outcomes and facility care standards.
