The physician serves as the primary care provider for a dedicated panel of geriatric patients in long-term care facilities (ALFs/ILFs) and private homes. Physicians are assigned a convenient geographic area where patients are located, with flexibility regarding whether house-call patients are included in the panel. The number of patients seen each day varies according to visit type, travel time, and patient needs. Dedicated schedulers create daily schedules and routes to support efficient visits and appropriate time with each patient. The role includes conducting health assessments, developing personalized care plans, evaluating patient conditions and responses to treatment, adjusting treatment plans, performing minor procedures and wound care as required and as settings permit, educating patients and families, offering guidance on lifestyle modifications and self-care, and coordinating with nurse practitioners, nurses, and other healthcare professionals. The practice uses a mobile-based EHR with AI functionality to reduce administrative time. On-call responsibility is phone triage only, is described as very low volume, and rotates among providers.
