About The Position

Conviva Primary Care, a subsidiary of Humana Inc., is hiring a Primary Care Physician for its clinic in Winter Haven, Florida. The physician will provide onsite outpatient care to adult and senior patients and report to the Associate Medical Director. The position is scheduled Monday through Friday, 8:00 a.m. to 5:00 p.m., for 40 hours per week.

The role uses a team-based, whole-person care model focused on patients’ physical, emotional, and social needs. It offers a lower daily patient volume to support personalized care, a collaborative and inclusive work environment, PTO, minimal call responsibilities, and dedicated CME time. The position includes a sign-on bonus of up to $60,000 and is eligible for a performance-based bonus incentive plan.

The clinic is located at 141 E. Central Ave., Winter Haven, FL 33880. The posted annual base-pay range is $255,000 to $298,500. Individual compensation may vary based on location and job-related qualifications.

Requirements

• Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO).
• Board certified or board eligible in geriatric medicine, family medicine, or internal medicine.
• Valid, unrestricted Florida license, or willingness to obtain one.
• Current, unrestricted DEA license, or willingness to obtain one.
• At least two years of experience in primary care, geriatric medicine, or managing chronic conditions.

Responsibilities

• Deliver comprehensive outpatient care to adult and senior patients.
• Maintain accurate medical records and documentation.
• Diagnose and manage moderately to complex medical conditions.
