About The Position

This is a travel, temporary assignment for a Physical Therapist in home healthcare - hospice. The role involves evaluating and treating patients to restore function and minimize disability within a defined territory including Sunnyvale and neighboring cities. Responsibilities include documentation, collaboration with an interdisciplinary team, and potentially supervising Physical Therapy Assistants.

Requirements

• One year of Home Health experience
• Ability to work in a home health/hospice setting and travel within the defined territory
• Strong documentation skills, including experience with skilled interventions and discharge planning
• Ability to perform initial admissions/evaluation visits and coordinate OASIS scoring within established timelines
• Effective communication and teamwork to collaborate with the interdisciplinary care team

Responsibilities

• Evaluate and treat patients using medically prescribed physical therapy programs to restore function and minimize disability
• Supervise Physical Therapy Assistants when appropriate; participate in initial admissions/evaluation visits
• Document observations, clinical findings, problems, skilled interventions/treatments, goals, and discharge plans
• Complete and timely documentation, including OASIS scores, and notify the Clinical Team Manager within established timelines
• Maintain thorough patient records and discharge planning, adjusting plans as progress is made
• Collaborate with an interdisciplinary team to deliver patient-centered care aligned with home health and hospice goals
