About The Position

Adoration Health is seeking a dedicated Home Health Physical Therapist to provide patient-centered care in Dublin, Georgia, and the surrounding coverage area. The role involves working independently with support from a team and providing care in patients’ homes. Full-time, part-time, and PRN schedules are available. A $15,000 sign-on bonus applies only to external applicants.

Requirements

• Bachelor’s degree in Physical Therapy from a college or university with an accredited Physical Therapy program.
• At least two years of appropriate experience as a physical therapist.
• Valid license in the state of practice.
• Current CPR certification.

Nice To Haves

• Community or hospice experience is preferred.
• New physical therapy graduates may be considered in select markets based on program availability.

Responsibilities

• Perform physical therapy evaluations and treatments based on physician orders, including recommending and procuring adaptive equipment.
• Establish measurable goals and develop treatment plans to help patients reach their maximum level of independence and function.
• Communicate with the treatment team, physician, patient, and family members as applicable.
• Maintain thorough and timely documentation of services to ensure proper billing.
• Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and related activities in accordance with the state practice act.
