About The Position
Adoration Home Health is seeking a dedicated Home Health Physical Therapist to provide patient-centered care in Pensacola, Florida. The role provides one-to-one care in patients’ homes, with flexible scheduling options and the ability to work independently while supported by a team. The position covers the Pensacola area and includes a $15,000 sign-on bonus for external candidates only. The Physical Therapist evaluates and treats patients according to physician orders, develops measurable goals and treatment plans, coordinates with the treatment team and patients’ families, documents services for billing, and supervises Physical Therapist Assistants and Physical Therapy Aides. The position is subject to a level 2 background screening.
Requirements
- Graduate of a Physical Therapist education program approved by CAPTE.
- Current Physical Therapist licensure in the state.
- American Heart Association CPR certification; certification may be offered and completed upon hire.
- Two years of experience in adult physical therapy.
- Excellent observation, verbal and written communication, problem-solving, and basic math skills.
- Physical therapy skills as specified by the competency checklist.
Nice To Haves
- Home health experience is preferred.
Responsibilities
- Perform physical therapy evaluations and treatment based on physician orders, including recommending and procuring adaptive equipment.
- Establish measurable goals and develop treatment plans to help patients reach their maximum independence and/or function.
- Maintain communication among the treatment team, physician, patient, and family members, as applicable.
- Maintain thorough and timely documentation of services provided to ensure proper billing.
- Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and patient-related activities in accordance with the state practice act.
