About The Position

Adoration Home Health is seeking a dedicated Physical Therapist to provide patient-centered home health care in Douglas, Georgia, serving Atkinson and Coffee Counties. The role involves providing one-to-one care in patients’ homes while working independently with support from a treatment team. The position is full-time, Monday through Friday, from 8 a.m. to 5 p.m. A $15,000 sign-on bonus is available to external candidates only.

Requirements

• Graduate of a Physical Therapist education program approved by CAPTE.
• Current Physical Therapist licensure in the state.
• American Heart Association CPR certification; certification may be offered and completed upon hire.
• Two years of experience in adult physical therapy.
• Excellent observation, verbal and written communication, problem-solving, and basic math skills.
• Physical therapy skills as specified by the competency checklist.

Nice To Haves

• Home health experience preferred.

Responsibilities

• Perform physical therapy evaluations and treatment based on physician orders, including recommending and procuring adaptive equipment.
• Establish measurable goals and develop treatment plans to help patients reach their maximum level of independence and function.
• Maintain communication among the treatment team, physician, patient, and applicable family members.
• Complete thorough and timely documentation of services to support proper billing.
• Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and related activities, in accordance with the state practice act.
