About The Position

Adoration Home Health is seeking a dedicated Physical Therapist to provide patient-centered home health care in Ashland, Alabama, covering Clay, Cleburne, Randolph, and Tallapoosa counties. The role provides one-to-one care in patients’ homes, with independent work supported by a team. The schedule is full-time, Monday through Friday, 8 a.m. to 5 p.m.

The Physical Therapist evaluates and treats patients in accordance with physician orders, establishes measurable goals, and develops treatment plans to support patients’ independence and function. The role also includes coordinating with the treatment team, physicians, patients, and families; maintaining timely service documentation; and supervising Physical Therapist Assistants and Physical Therapy Aides during patient care and related activities.

Requirements

• Bachelor’s degree in Physical Therapy from a college or university with an accredited Physical Therapy program.
• At least two years of appropriate experience as a physical therapist.
• Valid license in the state of practice.
• Current CPR certification that is maintained.
• New physical therapy graduates may be considered in select markets based on program availability.

• Community or hospice experience is preferred.

Responsibilities

• Perform physical therapy evaluations and treatments based on physician orders, including recommending and procuring adaptive equipment.
• Establish measurable goals and develop treatment plans to help patients reach their maximum level of independence and/or function.
• Maintain communication among the treatment team, physician, patient, and family members, as applicable.
• Maintain thorough and timely documentation of services provided to support proper billing.
• Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and patient-related activities in accordance with the state practice act.
