About The Position

Adoration Home Health is seeking a dedicated Physical Therapist to provide patient-centered care in patients’ homes in the Dublin, Georgia coverage area. The role involves evaluating and treating patients according to physician orders, developing individualized treatment plans, coordinating with patients and care teams, documenting services, and supervising Physical Therapist Assistants and Physical Therapy Aides. Full-time, part-time, and PRN schedules are available. A $15,000 sign-on bonus applies to external applicants only.

Requirements

• Bachelor’s degree in Physical Therapy from a college or university with an accredited Physical Therapy program.
• At least two years of appropriate experience as a physical therapist.
• Valid license in the state of practice.
• Current CPR certification that is maintained.

Nice To Haves

• Community or hospice experience is preferred.
• New physical therapy graduates may be considered in select markets based on program availability.

Responsibilities

• Perform physical therapy evaluations and treatment based on physician orders, including recommending and procuring adaptive equipment.
• Establish measurable goals and develop treatment plans to help patients reach their maximum level of independence and function.
• Communicate with the treatment team, physician, patient, and family members as applicable.
• Maintain thorough and timely documentation of services provided to support proper billing.
• Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and related activities in accordance with the state practice act.
