Requirements

Active Physical Therapist license in the state of New Hampshire
Doctor of Physical Therapy (DPT) degree or equivalent

Availability for a 13-week travel assignment starting 12/01/2026
Strong communication and teamwork skills to collaborate with an interdisciplinary team
Eligible to work in the United States

Responsibilities

Provide comprehensive physical therapy evaluation and treatment planning for residents in a skilled nursing facility setting.
Implement individualized treatment programs, including therapeutic exercises, gait training, functional mobility, and restorative modalities.
Reassess progress, document outcomes in the care plan, and adjust goals as needed in collaboration with the interdisciplinary team.
Educate residents and families on home exercise programs, safety, and discharge planning.
Communicate effectively with nurses, physicians, and therapy colleagues to optimize resident outcomes and ensure regulatory compliance.
