About The Position

This is a travel Physical Therapist position for a temporary assignment in a skilled nursing facility in Seattle, WA. The assignment is for 13 weeks, starting on 09/28/2026. The role involves performing comprehensive PT evaluations, developing and implementing individualized treatment plans, providing therapeutic interventions, monitoring patient progress, and educating residents and families. Requirements

Nice To Haves

Prior SNF or geriatric PT experience is preferred.

Responsibilities

Perform comprehensive PT evaluations for residents in the SNF setting and establish measurable treatment goals.
Develop and implement individualized treatment plans to improve mobility, strength, balance, gait, and functional independence.
Provide therapeutic exercise programs, gait and transfer training, ROM techniques, and pain management as appropriate.
Monitor patient progress, modify treatment plans as needed, and document outcomes accurately in accordance with policy and regulatory standards.
Educate residents and families on exercises, safety, and discharge planning; coordinate care with the interdisciplinary team.
