About The Position

This opening supports a licensed PT providing rehab services within a skilled nursing facility. The role involves performing initial and ongoing assessments of residents to determine rehabilitation needs and establish measurable goals. The Physical Therapist will develop and implement individualized physical therapy plans of care, delivering evidence-based interventions and modalities as appropriate. Direct therapeutic sessions will be provided, progress monitored, and treatment plans adjusted to optimize function and safety. The position requires meticulous documentation of evaluations, progress notes, treatment plans, and discharge planning in accordance with regulatory and facility requirements. Collaboration with interdisciplinary team members, including nursing, OT, speech therapy, and physicians, is essential for coordinating care and supporting resident outcomes. The role also includes contributing to discharge planning and making referrals to community resources or follow-up services as needed.

Requirements

  • Current Physical Therapist license in New Mexico (or eligibility/credentialing as applicable)
  • Doctor of Physical Therapy (DPT) or equivalent PT degree from an accredited program
  • Experience in geriatric or skilled nursing rehabilitation preferred
  • Ability to work effectively in a long-term care setting and within a multidisciplinary team
  • Strong clinical documentation and communication skills
  • Availability to commit to a 13-week travel assignment starting 08/31/2026
