About The Position

Physical Therapist needed for a skilled nursing facility in Moses Lake, Washington. This travel temporary assignment starts on 08/31/2026 and runs for 19 weeks. You will earn $2,598–$2,658 per week with 35 guaranteed hours per week. The shift is 5x8 days (08:00–16:30). Work in a long-term care setting within a licensed skilled nursing environment as a travel clinician with Healthforce Healthcare LLC.

Requirements

Active Physical Therapist license in Washington state
Doctor of Physical Therapy (DPT) degree or equivalent
Prior experience in a skilled nursing facility or geriatric rehabilitation setting preferred
Strong clinical assessment, treatment planning, and documentation skills
Ability to work independently within a multidisciplinary team and commit to 35 guaranteed hours per week

Responsibilities

Perform comprehensive PT evaluations and develop individualized plans of care for residents in a geriatric, skilled nursing setting.
Implement evidence-based therapeutic interventions to improve mobility, strength, balance, gait, and pain management; monitor progress.
Reassess patients regularly, adjust treatment plans, and document changes in the electronic medical record.
Collaborate with an interprofessional team (nursing, therapy aides, physicians) to optimize patient outcomes and discharge planning.
Educate residents and families on exercise programs, safety strategies, and home-program plans; ensure patient safety during activities and transfers.
Maintain accurate documentation of treatment sessions, outcomes, and any changes to goals or frequency of care.
