About The Position

Physical Therapist needed for a Skilled Nursing Facility in Nashua, New Hampshire, United States. This is a 13-week travel assignment with a start date of 08/31/2026. The role involves conducting evaluations, developing and implementing therapy programs, reassessing progress, documenting care, and providing patient education. Requirements

Active Physical Therapist license in New Hampshire or eligible for licensure in NH
Ability to practice in a skilled nursing facility setting
Strong documentation and communication skills; comfortable working with a multidisciplinary team
Available for a 13-week travel assignment with a 40-hour work week

Responsibilities

Conduct initial and ongoing PT evaluations to determine resident needs and goals.
Develop and implement individualized therapy programs to improve mobility, strength, balance, and functional independence.
Reassess progress, adjust treatment plans, and set measurable outcomes in collaboration with the interdisciplinary team.
Document evaluations, treatment plans, progress notes, and outcomes in accordance with regulatory and facility standards.
Provide patient and caregiver education, safety training, and fall-prevention strategies.
Collaborate with nursing, medical staff, and other therapy disciplines to coordinate comprehensive resident care.
