About The Position

The Physical Therapist works in a Skilled Nursing setting, evaluating and treating patients recovering from orthopedic surgeries, neurological events, cardiac conditions, and general deconditioning. The therapist develops individualized treatment plans and collaborates with nursing, physicians, families, and the interdisciplinary care team to help patients achieve meaningful functional outcomes and return to their highest level of independence. Caseloads are described as manageable, and clinicians are supported in exercising their clinical judgment and providing evidence-based care.

Requirements

• Doctor of Physical Therapy (DPT) or Master's degree from an accredited physical therapy program.
• Current or eligible for a New York State physical therapy license.
• Passion for geriatric rehabilitation and functional outcomes.
• Strong communication and teamwork skills.
