About The Position

In this Skilled Nursing Facility (SNF) role, the Physical Therapist evaluates and treats patients recovering from orthopedic surgeries, neurological events, cardiac conditions, and general deconditioning. The therapist develops individualized treatment plans and collaborates with the interdisciplinary care team, including nursing, physicians, and families, to help patients achieve meaningful functional outcomes and return to their highest level of independence. The role offers a manageable caseload and respect for clinical judgment.

Requirements

• Doctor of Physical Therapy (DPT) or a Master's degree from an accredited PT program.
• Current or eligible for New York State Physical Therapist licensure.
• Passion for geriatric rehabilitation and functional outcomes.
• Strong communication and teamwork skills.
