About The Position
BAYADA Home Health Care is hiring new graduate Physical Therapists to provide one-on-one care to adult and geriatric clients in their homes in Wilmington, Delaware. The role includes structured orientation, dedicated clinical support, and mentorship from experienced physical therapists to help new graduates transition to independent practice. Physical therapists treat one patient at a time and support clients in remaining safe and independent at home. The position offers flexible scheduling and autonomy, manageable territories, and opportunities for clinical education and professional development.
Requirements
- Graduate of an accredited Doctor of Physical Therapy (DPT) program.
- Licensed or license-eligible Physical Therapist in the state of practice.
- Excellent communication and interpersonal skills.
- Valid driver's license and reliable transportation.

Responsibilities
- Evaluate and assess patients' functional status and mobility.
- Develop individualized treatment plans to maximize independence and safety.
- Provide skilled therapeutic interventions in patients' homes.
- Educate patients, families, and caregivers on exercises, mobility, fall prevention, and home safety.
- Collaborate with physicians, nurses, occupational therapists, speech therapists, and other healthcare professionals.
- Complete timely electronic documentation while maintaining high standards of patient care.
