About The Position

This is a 12-week travel assignment for a Physical Therapist in a home health care setting. The role involves performing physical therapy evaluations, developing and implementing plans of care, and educating patients and their families. The position requires documentation using EPIC and familiarity with OASIS data. The coverage territory includes NW Philadelphia and Montgomery County.

Requirements

• Recent, relevant home health experience
• EPIC experience and OASIS familiarity
• Experience with start of care and revisits
• Ability to fulfill assignments in the NW Philadelphia and Montgomery County area
• Strong communication and documentation skills; dedicated to patient-centered care

Responsibilities

• Perform physical therapy evaluations in the home setting and develop a plan of care aligned with patient goals.
• Deliver PT interventions per the plan of care, monitor patient progress, and adjust treatment as needed.
• Conduct start of care visits and follow-up revisits; document assessments and progress in the designated health record system.
• Utilize EPIC for documentation and case management; collect and manage OASIS data as required.
• Educate patients and family members on home exercise programs, safety, fall prevention, and activities of daily living.
• Collaborate with an interdisciplinary team to coordinate care, communicate progress, and ensure regulatory and quality standards are met.
