About The Position

This is a 12-week travel assignment for a Physical Therapist – General in Bala Cynwyd, Pennsylvania. The role is in a Home Health setting and guarantees 40 hours per week with a daytime shift from 8:00 a.m. to 4:30 p.m. The weekly pay range is estimated between $2,549 and $2,742.

Requirements

Active Physical Therapist license in Pennsylvania or eligibility for PA licensure.
Recent home health experience.
EPIC EMR experience and proficiency with OASIS documentation.
Demonstrated ability to perform Start of Care and revisits in a home health setting.
Ability to commit to 40 hours per week on a 5x8 daytime schedule.
Willingness to travel within the assignment area to provide home visits.

Responsibilities

Perform skilled physical therapy evaluations and develop individualized plans of care for home health patients.
Deliver therapeutic interventions focused on gait, balance, strength, ROM, and activities of daily living.
Monitor patient progress, update treatment plans, and document outcomes in the electronic medical record.
Conduct Start of Care visits and periodic revisits, adjusting plans as needed to meet patient goals.
Educate patients, families, and caregivers on safety, transfers, and home exercise programs.
Collaborate with the interdisciplinary team to coordinate care and ensure smooth transitions and discharge planning.
