Physical Therapist (PT) – Full-Time | Skilled Nursing & Rehab
About The Position
We are seeking a motivated and compassionate Physical Therapist to work full-time in Cadiz, Ohio, in a rehabilitation or skilled nursing facility setting. The role focuses on helping patients improve mobility, regain independence, and achieve rehabilitation goals. New graduates and experienced therapists are encouraged to apply. The Physical Therapist evaluates patients, develops individualized treatment plans, delivers therapeutic interventions, monitors progress, and adjusts care as needed. The role also includes timely and accurate documentation, collaboration with the interdisciplinary team, and education for patients, families, and caregivers.
Requirements
- Current Ohio Physical Therapist license or eligibility to obtain licensure.
- Graduate of an accredited Physical Therapy program.
- Strong communication, organizational, and clinical skills.
- Commitment to quality patient care and positive rehabilitation outcomes.

Responsibilities
- Evaluate patients and develop individualized treatment plans.
- Provide physical therapy interventions to improve strength, balance, mobility, and functional independence.
- Monitor patient progress and adjust treatment plans as needed.
- Document evaluations, treatments, and progress accurately and in a timely manner.
- Collaborate with the interdisciplinary team to support positive patient outcomes.
- Educate patients, families, and caregivers on therapeutic exercises and safe mobility techniques.
