About The Position

Elderwood at Hamburg is hiring a Physical Therapist to provide skilled therapy in a skilled nursing setting. Therapists are Elderwood employees and part of the building’s care team, rather than working through a contract rehabilitation company. The role offers a varied caseload that may include orthopedic rehabilitation, neurological and cardiac care, post-hospital recovery, fall prevention, and long-term care. Therapists participate in care planning and clinical discussions, contribute recommendations to residents’ plans, and work with residents over time to support continuity and observe their progress. The stated productivity target is 85%. The position is available full-time, part-time, or PRN, with flexible scheduling described as a weekday schedule and limited weekend rotation. Pay is up to $45.00 per hour based on experience; the posted salary range is $33.00–$45.00 per hour. New graduates are welcome, and the employer offers mentorship and structured orientation.

Requirements

Graduation from an approved school of physical therapy.
State license to practice physical therapy.
Ability to direct others.
Ability to read, write, speak, and understand English at an intermediate or more advanced level.

Nice To Haves

Two years of experience in physical therapy is preferred.

Responsibilities

Provide skilled physical therapy evaluations and treatments in accordance with physician orders.
Develop and update individualized care plans and therapy goals.
