About The Position

In its Skilled Nursing setting, the Physical Therapist evaluates and treats patients recovering from orthopedic surgeries, neurological events, cardiac conditions, and general deconditioning. The therapist develops individualized treatment plans and collaborates with nursing, physicians, families, and the interdisciplinary care team to help patients achieve functional outcomes and return to their highest level of independence. The role offers a manageable caseload and respect for clinical judgment. Structured mentorship is available for new graduates.

Requirements

• Doctor of Physical Therapy (DPT) or a Master's degree from an accredited physical therapy program.
• Current New York State physical therapist license or eligibility for one.
• Passion for geriatric rehabilitation and functional outcomes.
• Strong communication and teamwork skills.
