About The Position

This is a 13-week travel assignment for a Physical Therapist – General in Clearlake, CA, United States. The role is based in a Critical Access Hospital setting and guarantees 40 hours per week. The position involves planning and conducting treatment programs to restore physical function, developing individualized treatment plans, performing comprehensive evaluations, and supervising assistants. The start date for this assignment is October 12, 2026.

Requirements

Active Physical Therapy licensure in California (or applicable states) required
Doctor of Physical Therapy (DPT) degree required; or foreign equivalent if licensure obtained after January 2016
CPR/BLS certification required
Valid driver’s license; at least 21 years old
Minimum two years of recent physical therapy experience
Knowledge of HCHB and OASIS; EPIC experience; at least 1 year of home health experience; No on-call requirement; mileage reimbursement paid at government rate

Responsibilities

Plan and conduct segments of treatment programs to restore and improve physical function; work with an interdisciplinary team
Develop and implement individualized treatment plans using evidence-based PT interventions; monitor progress
Perform comprehensive evaluations, set goals, and communicate discharge plans; document progress and outcomes
Maintain accurate clinical documentation within established deadlines; ensure quality records
Supervise assigned assistants and participate in care coordination and discharge planning
Adhere to safety, infection control, and facility policies while supporting quality improvement initiatives
