About The Position

This is a travel assignment for a Physical Therapist in a Short Term Acute Care Hospital setting in Sonora, California. The contract duration is 13 weeks, with a start date of 09/28/2026. The role involves planning and conducting treatment programs to restore and improve patient functions, serving as a co-leader in interdisciplinary patient groups, performing evaluations, implementing therapy interventions, developing discharge plans, supervising assistants, and participating in care coordination.

Requirements

Doctor of Physical Therapy (DPT) degree.
Physical Therapy licensure in the state of practice (CA) required.
CPR/BLS certification from an approved provider required.
At least two years of recent experience in the field.
Valid driver’s license (age 21 or older) required.
EPIC experience.
Current license to practice physical therapy in applicable states of practice.

Responsibilities

Plan and conduct segments of treatment programs designed to restore and improve physical, social and mental functions, aligning with department objectives.
Serve as co-leader in an interdisciplinary approach to patient groups, collaborating with other care professionals.
Perform comprehensive evaluations, establish individualized treatment plans, and implement therapy interventions using established protocols.
Develop and communicate individualized discharge plans; ensure documentation is accurate and timely.
Supervise and delegate tasks to assigned assistants, and maintain thorough clinical documentation within required timelines.
Participate in ongoing care coordination and patient progress communication with the care team and families.
