Requirements

Active New York state Physical Therapist license
BLS certification
Prior experience in a skilled nursing facility
Weekend availability, specifically Saturdays
Ability to work effectively within a skilled nursing facility setting and follow established policies and procedures

Responsibilities

Perform initial assessments and develop individualized treatment plans for residents.
Provide skilled physical therapy interventions focused on mobility, gait, balance, functional mobility, and activities of daily living.
Reassess and adjust treatment plans based on resident progress and goals.
Document evaluation findings, plan of care, progress notes, and discharge summaries in the resident chart per regulatory and facility policies.
Collaborate with nursing, therapy aides, and other care team members to coordinate care and support discharge planning.
Educate residents and families on home programs and safety strategies to promote continued progress.
